As part of the national dementia audit, we are seeking feedback from carers who have visited a loved one at King’s who has dementia and been cared for on a ward – anytime between June-September 21st 2018.
We ask for any carers to complete the survey they can do so by going online at www.CARERQ.org.uk or they can get in touch with our team and we can collect their answers over the phone.  It needs to be specific to this time period and from a carer visiting King’s.
