Integrated care pilot: seeking the views of people aged 75 or over, and their carers

Kings College Hospital (KCH) have launched a new service. The Integrated Care Pilot (ICP) aims to truly involve the people it aims to benefit by defining a better way for primary, secondary, social and community care to work together effectively, for the benefit of patients and the people who care for them.

KCH are running a series of discussion groups and one-to-one discussions with people from Lambeth and Southwark who are over 75 years old (and/or their carers). This could be as informal as a chat over the telephone or a face- to-face meeting (we are very happy to come to participants if possible). We are also looking to recruit members to our reference group. Feedback received from these discussions will directly inform the work being carried out in our clinical design groups and the programme as a whole.
