Reevaluating the needs of Carers in 2016

To be entered into the prize draw to win a first prize of £150 of theatre tokens you must include your name and address and answer all questions on the form. Closing date for entires is Tuesday 31st January 2017.

There are also 50 other prizes of £45 payments for pamper treatments. The payments can be used for a range of sessions from massage, to hair care, manicures, pedicures and haircuts.

The first set of questions relate to you and your caring role.

The second set relate to the overall social context and wider issues relating to carers and their rights.

All information recorded on this form will remain anonymous.
