Job description:
Carrying out in house carers assessments.
Co-ordinate and maintaining the package of support including personally providing support and accessing and monitoring other resources, including:
- advocating on behalf of service users with other agencies
- welfare benefits advice and budgeting skills
- accessing resources from statutory and voluntary agencies
- support related to individuals needs
Provide information and consult service users about services provided; actively encourage service user participation through facilitation of service user meetings and other activities.
Implement procedures for service user selection and allocations in collaboration with the CNW team, including:
- seeking referrals
- interviewing and assessing prospective service users
To work effectively as a project member, including:
- sharing information
- taking on project wide responsibilities
- providing cover for other project members
- contributing to the development of the service
- working agreed hours which may include evenings and some weekends
Develop effective and professional working relationships internally and externally promoting a positive image of the service and of Epic Trust.
Plan and organise own work and follow office procedures and administrative systems, including keeping service user’s files up to date.
Meet key performance targets including Carers Assessments and service user-focused monitoring.
Keep your professional knowledge updated e.g. of legislation, welfare rights, procedures, resources available for service users.

The Princess Royal Trust Carers Service provides an advice and information service to unpaid and family carers across Berkshire
Working as part of our management team, this post plays a key role in the leadership, management and continuing development of our service in the Boroughs of Reading and Wokingham, managing staff, service development and change and holding responsibility for limited direct work with carers.
The post is based in Reading with travel across Reading and Wokingham. Applicants must have use of own car.
We offer 29 days annual leave plus bank holidays pro rata for part-time hours and a contribution towards retirement planning of 4% of salary. Work travel expenses are paid at 40p per mile.

Salary: £27,850-£29,434 per annum incl. IFLW NJC Scale 32-34Hours: Full Time 36 hours per week
Job description:
We are looking for a keen and committed person to provide an effective communication network between our organisation and other agencies, across Surrey and nationally, on carers’ issues. You will also ensure all carers have a voice by gathering their views for strategic planning as well as disseminating information to carers and those who support them in the community, developing and using a variety of different methods to reach carers in all circumstances.
Candidates will be experienced and pro-active professionals, who are able to work in partnership with voluntary and statutory agencies as well carers. Experience of gathering and disseminating information, developing a media and communication strategy and working on publicity materials is essential. The ideal candidate will also need the ability to summarise complex issues into formats suitable for wider consumption.
Candidates must be IT literate, have the ability to work to deadlines, work without direct day to day supervision and be able to engage and enthuse others in the work of the organisation.
This position will be based at our Guildford Office and will require travel throughout the county.
